template for reporting Vulnerability Testing results, fw request form

Hi All,

Since many of you seem quite experienced, I was hoping you may 
be able to share your preferences in documenting vulnerability / 
penetration testing results to management. I will be using Nessus and nmap
to do the testing. Is there a particular format / template that most of you
use for this purpose?

Also, if you anyone has a particular form for firewall requests (i.e.,
opening ports, etc.) that would be great.
