Security Basics: fault logs

Hi all, trying to develope a fault log database. 

I have the following filed already, - 
Event detected -    (what, when, by whom) 

Impact Assessed / Observed-  (what when by whom)

Team/Vendor Notified-  (who, when, )
ticket/call reference

Correspondence with team/vendor (all info per message)

Escalation with team/vendor - (who, when, reference)

Resolution Date- (when, what was done, end result)

Notes - (useful information for future reference)

if you have any ideas on usefull filed that i have nt included pls let me know . Thanks.
