In my experience, the biggest worry is whether the 
   a) kid reselling domains out of Mom's basement, or
   b) single sole proprietor, or 
   c) any other tiny company 

...for some reason stops paying their bills to the upstream registrar
thereby:
     1) making your domain inaccessible/redirected, 
     2) leaving you without any control panel access to repoint
        your domains elsewhere
     3) leaving no one around at the losing registrar who has a
        whole lot of interest in approving your domain's transfer
        to another registrar

As such, accountability and critical mass therefore are likely among
the reasons you'll see most companies using one of the larger well
known registrars like GD and NS.

I've helped people whose registrar (a serial entrepreneur type that
seemed to have tried everything) simply forgot to pay the bill but did
pay up once I helped the guy track the registrar down at his place of
employment (with the help of flickr geotagging of an office party).  I
also helped someone who had hosting and registration service at the
same mid-size but sole proprietor company recover after that sole
proprietor had a heart attack and passed away.
