1. How are you making up loss of revenue on IPv4 assignments?

If revenue from IPv4 assignments is an issue, then the solution is to adjust your business model to not depend on 
that revenue.  As an ISP, the business is to ship bits around.

To that end I've never charged for IPv4, either.
