I figure 1% is a good threshold. If we're less than 1% done we shouldn't
have very good scan estimates anyways. This threshold can, of course, be
changed.

Thanks, this looks great!  I applied it, except I just removed the ETC
estimate rather than printing '??'s for too-early estimates (I worry
they might confuse people).

o Fixed a bug related to bogus completion time estimates when you
  request an estimate (through runtime interaction) right when Nmap is
  starting.a subsystem (such as a port scan or version detection).
  Thanks to Diman Todorov for reporting the problem and Doug Hoyte for
  writing a fix.
