Nmap Development: [PATCH] tcpip.cc and protocol.cc fixes

hey guys

Here are a couple of fixes for bugs I came across while porting a
patch to 4.21ALPHA1.

1) Fixes a bug in build_icmp_raw() where the TTL argument is ignored
and the packets were always build with o.ttl

2) In nmap_getprotbynum()  nmap_protocols_init() was commented out
causing the function to fail in certain scenarios.
