No, there is no difficulty about any string. I'm aware of this notice about
"%(num)d ports open on %(host)s" % { "num": num, "host": host } rather  
than "%d ports open on %s" % (num, host) but imho it's only usefull  
when it deals with "general translation" jobs : I mean, when the  
translator doesn't know about the product he's translating. He needs  
to understand the variables.
But when the translator is also a user, he knowes almost all the  
strings, so he understands what it's about.
So, it's not about the _place_ of the words. The point is not to  
forget any variable in the translated form: it's easy to forget a "%s"  
at the 1002nd line, wich leads to a weird "number of format  
specifications in 'msgid' and 'msgstr' does not match" that can easely  
cause a headach !
The instructions at  
http://nmap.org/book/zenmap-lang.html#zenmap-lang-new are accurate,  
but I must say that there is an easiest way to do the job : I suggest  
any translator to use Poedit ( http://www.poedit.net/ ). This soft can  
do everything from the .pot file to the final .mo file.
The .po is generated, and the translator just has to deal  
with...translation : only the strings to translate are given. Here is  
a screenshot to clarify this :

There is just one trap about .po that translators must know (and imho   
this should be added to the instructions): if you don't use a .po  
Editor, be very carefull about your favorite text editor !
Some of them can't deal with UTF-8 and you get the (not so...)  
self-speaking "lang.po:1:2: parse error " because they add the "UTF-8  
BOM" string (see  
http://article.gmane.org/gmane.comp.gnu.utils.bugs/11139 for details).  
And .po doesn't like it. At all.
Last point, Translators :
Translation is easy: of course you know your own native langage, and  
after all... all you need is to understand english.
With the approriate tool there is absolutely no technical difficulty.  
So even if you are not a "geek", don't be afraid by those strange  
files .pot, .mo and so on. Just read the text, and translate !
And at any time you can ask for help and backup here. It's a cool list!
