I think it's possible to create a virus that infects win32 binaries that
contains images. Part of the code would be on the executable area, and the
other part could be in a Icon image. This would make easier for larger
viruses to hide themselves in an bigger executable file.

straight ResEdit on the file who's icon you wish to modify.  Much the same
way you can alter the icon of a win32 binary with similar reshacking
programs.  When the .res file is added during compilation its a standard
.ico file added as static data to the binary.
