It would list each device that the sysadmin is responsible for, and a number of categories for kinds of maintenance, with free-form entry fields and would log all of this to a SQL database for reporting. A wiki might also fit the bill. I use moin-moin wiki running under Apache+SSL with basic user authentication as a development journal/admin journal. It doesn't have fine-grained access control but is very easy to use and keep updated and includes search functions.
