Get a palm ( any old one will do ), and get Passwords Plus from Dataviz. You 
get a copy for your PC so there's a backup you can always access, and 
there's a copy for the Palm so it's mobile. Everything's encrypted, but it's 
easy to get at.  If you lose the palm, your passwords and other information 
are secure, but you can easily replace it and reinstall all of your 
information.

Entries are sortable by category, and you can setup templates for new 
entries with your choice of fields.

I love mine.

I'm sure there are other similar solutions, including some for Pocket PC if 
that's your choice of mobile OS.
