Does anyone know if a list is being kept of the types/causes of these
data-breaches?

- Network Intrusion
- Insider
    - Malicious
    - Careless
- Loss by third party (sub-contractor, partner etc...)
- Spyware/malware/virii
- Poor Physical Security Measures
    - Loss of hardware
    - Loss of printed data
    - Mis-communication of data to a third party
