I've used Password Manager Pro a lot.  It is easy for the user and admin, automates updating passwords for a TON of 
different platforms (Windows, Cisco, Linux, etc.), reminders, and kept good logs.  I do have to say that a few years 
back (say 3 now) the server needed to be rebooted monthly, but that seemed to settle down in the version from 2 years 
ago.
