Hi all,
After finishing every security audit, I always found it dificult to
categorize various vulnerabilities properly.
And each audit taught me few chapters. 
So, the presentation of report has evolved into pretty professional
format.

I am attaching a template of report. It might help some of us to avoid
reinventing the wheel.
