I have a copy of it (disclaimer: I didn't create it,
but I _was_ a reviewer of it).  Unfortunately, I'm not sure
if I can give it out or not!!   I will see if I can get permission
to post it (or something based on it) to the web.
Alternatively, you can make a Freedom of Information Act
request to try to get it.
