Database ERROR:Database ERROR:Duplicate entry

I have searched through the archive, so far nothing has really resolved
the issue.
Below are the tables referenced in a previous post.
Any clues to resolve the issue would be greatly appreciated.
Current setup.

mysql  Ver 12.20 Distrib 4.0.13, for portbld-freebsd4.8 (i386)
apache+mod_ssl-1.3.27+2.8.14
mod_php4-4.3.3.r1_1,1
