The most common mistake when someone uses tor is not to make DNS
resolutions go through Tor, too. They call it "DNS leaking". More info
here: http://wiki.noreply.org/noreply/TheOnionRouter/TorFAQ#head-e0b83d1dae14b598d805a972c1e9b07f717a9be1

I run an anonymous proxy server, so I had to implement a couple of
hacks here and there (all very well documented, check out
tor.eff.org). Basicly, you tell Squid not to use internal nameservers,
but to use an external dns resolver. This resolver gets the requests,
but does the resolution through tor. A bit of extra delay, but good
enough!

See http://linux-consulting.buanzo.com.ar for more details.
