We switched from using our own internal processes (blacklists) to block spam and malicious infections coming through e-mail to using the hosted MXLogic solution that several others have mentioned. The service has been fantastic in eliminating all sorts of unwanted communication (I think when I last checked, about 60% of the mail we had been receiving has been blocked as classified spam before even touching our systems) and offers customization to both the administrator(s) and user community.
