We switched from using our own internal processes (blacklists) to block spam and malicious infections coming through 
e-mail to using the hosted MXLogic solution that several others have mentioned.  The service has been fantastic in 
eliminating all sorts of unwanted communication (I think when I last checked, about 60% of the mail we had been 
receiving has been blocked as classified spam before even touching our systems) and offers customization to both the 
administrator(s) and user community.  The drawback is that there is a monthly charge per user, which can add up to an 
unexpected chunk of one's IT budget if not properly planned out.
