maybe this answers my questions. I am not sure. 

Is there any clear documenation of what is going on here? 

Does this mail system connect to some other smarthost, and log into
the smarthost using that certificate rather than a username and password?

If it's not doing that, and it sure doesn't sound like it is, you're fine.
